This scheme is introduced the Chief Minister of the Uttar Pradesh Yogi Adityanath. Ineligibility For Scheme

• If any of the beneficiaries who are already getting the benefits from Ayushman Jan Arogya Yojana are not eligible for scheme.
• If any of the beneficiaries who registered with CM Jan Aarogya Abhiyan are also ineligible.
• Applicant’s who already insured with any of the government-run health care scheme of state or centre are also not eligible to get benefit of this scheme.

• Government launched the dedicated portal for this scheme i.e. sects.up.gov.in
• Here you see the home page of the portal, where you get the multiple option but you are first time to apply for this scheme, then you need to click on link “Apply For Shikshak Cashless Chikitsa Card.
• Now here You will get the Form where you only have to enter the Mobile Number which is linked with the Aadhaar card number.
• Now solve the Captcha Code and Enter the same. Click to Generate OTP.
• After entering the OTP in the given Place, Aadhar Card authentication complete and you see that application form of the scheme open in front of you.
• Now start adding the personal details and then upload all the documents with the form.

• Then application status form open in front of you.
• You can Check your status with the help of you Aadhar Card Number.
• Enter the 12 Digit your Aadhar Number and then enter the Security Code shown in your screen.
• Submit all the details and then Application Status appeared in front of you.

CM Teacher Cashless Medical Scheme Health Card Download

• All the beneficiaries who successfully submit their registration form of the scheme and then check their status.
• If your status appeared verified or successfully submitted, then later you will be eligible to Download the Health card of the scheme.
• To Download your CMTCTS Health card you need to open portal and then go to the Link of Print State Health Card available there.
• Before Download it you also need to complete your E KYC and then download Card.
• To Download Card enter the registered Mobile Number and then verify the same with OTP.
• Then have to select Scheme Name and District Name.  if you already complete the E KYC of the scheme, then you get the link of Download State Health card.
• Download the card in pdf format and you can take the printout of the card to save the same in future.
